New Patients

Welcome to San Dimas Medical Group, Inc.

Thank you for scheduling your appointment with us.  In an attempt to make your experience with us as effortless as possible, we offer the following suggestions and guidelines:

1.  Our office utilizes a state of the art electronic medical record.  The good news for you is that your information will be readily available to our providers 24/7 as well as improves the speed in which we receive your information from outside sources such as lab results.  The bad news is that it will take a few extra minutes to enter your information into the electronic medical record the first time.  You can assist us in this process and reduce your wait by providing us your new patient paperwork in advance.  You can complete these forms online by registering on our patient portal or mailing the forms to our office 48 hours prior to your appointment.  This will give us time to set up your record prior to you appointment.  If you do not provide this information in advance, your appointment may be delayed as we update your record at the time of the appointment.
2.  The forms you will need to complete can be found here.  Please be sure to complete these prior to the day of your appointment.
3.  Please arrive 15 minutes early for you first appointment.  This will allow us to check for any missing information as well as update your insurance information.

4.  We charge for “No Show” appointments.  You must call 24 hours in advance to cancel you appointment with us.

5.  We have our own onsite Lab Draw Station. Our draw station sends your labwork to either Physicians Automated Laboratory or LabCorp.  It is your responsibility to notify us in advance if your insurance company utilizes a different lab.  We will facilitate the sending of your specimens to that lab as long as we have the information in advance.

6.  If you are under the age of 18, please complete the Minor Consent Form.
